Indocyanine green angiography-guided photodynamic therapy with half-dose verteporfin for patients with chronic central serous chorioretinopathy |

PURPOSE To retrospectively study the effects of ICG-guided half-dose photodynamic therapy (PDT) with verteporfin(3 mg/m2) for patients with chronic central serous chorioretinopathy (CSC).
METHODS Twenty-three eyes of 20 patients with chronic CSC were enrolled in this study. ICG angiography was used to diagnosis CSC and localize the choroidal hyperpermeability of the disorder. ICG-guided PDT with half-dose verteporfin was applied to areas of choroidal hyperpermeability. Best corrected visual acuity (BCVA) and central retinal thickness (CRT) detected by optical coherence tomography (OCT) were evaluated before and after treatment.
RESULTS The vision improved in sixteen eyes (69.6%) and remained stable in seven eyes (30.4%) during a mean follow-up of 7.7 months (4-15months). The mean BCVA improved from 0.45±0.23 (ranging from 0.12 to 0.8) before treatment to 0.58±0.32 (ranging from 0.2 to 1.2) (P < 0.05) 3 months after PDT treatment, and to 0.64±0.29 (ranging from 0.3 to 1.2) (P < 0.05) at the last follow-up visit. Half-dose PDT guided by ICG was associated with complete resolution of the subretinal fluid in twenty eyes (87.0%) of 18 patients and incomplete resolution in the remaining three eyes (13.0%). And at the end of the follow-up, all eyes showed complete resolution of the subretinal fluid. The mean CRT was reduced from 297.9 ± 163.1µm before PDT to 207.3 ± 64.2µm (P < 0.05) 1 month after PDT, 192.3 ± 47.7µm (P < 0.05) 3 months after PDT and 200.1 ± 38.7µm (P < 0.05) at the last follow-up visit. A negative relationship between BCVA and CRT was found before and after PDT treatment in chronic CSC. During the treatment, no patient had any treatment-related side effects.
CONCLUSIONS PDT with half-dose verteporfin(3 mg/m2) guided by ICG seems to aid in the resolution of the subretinal fluid in patients with chronic CSC. The improvement of BCVA might be attributed to a rapid reduction in CRT due to the resolution of the subretinal fluid.
